Our short-term rehabilitation and long-term skilled nursing programs offer therapeutic recreation, rotating activity calendars and activities. Credentialed specialists will help residents with the following services:

What is long-term rehabilitation?

Long-term rehabilitation provides people with serious injuries or illness with 24-hour skilled nursing care in a residential setting, providing them a higher level of care, such as one-on-one assistance with meals and complete care for activities of daily living (dressing, medication management, bathing, etc.). Additional services provided in long-term rehabilitation include physical therapy, occupational therapy, speech therapy, diabetes management, medication management, wound care, cardiac care, IV/antibiotic therapy, hospice and palliative care as well as social services.

What is short-term rehabilitation?

Short-term rehabilitation helps residents recover from a hospital stay. It provides individualized rehabilitation and nursing therapy for accidents, strokes, hip fractures, joint replacement and other skilled nursing needs. Our goal is to build residents’ strength and skill levels, helping them adapt to new challenges as they progress through treatment toward the fullest and most independent life possible.

Long-term or short-term rehabilitation at PPH.

PPH is much different from other rehabilitation centers or skilled nursing in Philadelphia. We offer a beautiful and serene setting for providing the highest quality of care. A rehabilitation coordinator assists every patient through the continuum of care and into the community by assessing their level of care, communicating needs to families and coordinating discharge plans. The interdisciplinary team collaborates to ensure each resident is receiving the optimal level of nursing care while following the mission and values of PPH.

Q: Is skilled nursing and rehabilitation covered by Medicare?
Medicare does cover skilled nursing care provided in a skilled nursing environment if the person has Medicare insurance. The exact requirements, services covered, length of time and conditions may vary. Q: What kinds of activities are available for long-term and short-term rehabilitation residents?
Our Pathways program has certified recreational therapists to schedule and lead a range of activities across varied interests. Our activity calendars are designed to promote independence, community involvement and education, and to improve self-confidence. Some of these activities include trivia, musical entertainment, fitness games, pet visits, word games and much more.
